Fernville Surgery Patient Participation Group:

Help us improve our service to patients! We have formed a Patient Participation Group to make sure we can consider patients' views when developing services at Fernville. We need as many people as possible involved. But you can help by doing no more that completing a very brief survey every few months.

How can you help?

There are two ways to be involved:

• Become part of the small steering group, which meets in the Surgery every 6-8 weeks.

• Or just register to take part in occasional surveys on-line.
