Access to on-line services:
You can now book and
cancel appointments, and request your repeat prescriptions on-line. If you
would like use these services please click here to
download an
application form.
The completed form
should be sent to:

Alternatively you can
request a form by e-mail, or collect one from the surgery. You can
send the completed form by post or hand it in at the reception desk.
When we have processed your application, we
will send you your PIN number by post. When you have received your PIN
number please click below to complete the registration process and each time
you want to use the service.
Booking appointments on-line:
We offer a selection of appointments with each Doctor and each member of our Practice Nursing Team, bookable via Emis Access up to 7 days in advance.
Please help us provide the best possible service to you by booking an appointment with the most appropriate member of the team.
Practice Nurse - routine tests and treatments (dressings, cervical smears, injections, etc.)
Nurse Practitioner - minor illnesses (chest and urine infections,
abdominal pain, etc) and pill checks.
Our Nurse Practitioners can prescribe
medication.
Doctor - more complex problems.
Patients tell us that the on-line booking service is extremely convenient and easy to use. Unfortunately, a small number of patients have been inconvenienced when they have booked an appointment with a clinician who is not qualified to deal with their problem. If you need any advice regarding appointments, please contact our Reception Team (01442 213919) who will be very happy to help you).
Requesting repeat prescriptions on-line:
You can request items from your list of repeat medications through Emis Access. However, please note this service does not allow you to ask for new medication, or items that may have been prescribed to you in the past but are no longer on the repeat list.
Emis Access also provides links to information about the repeat medications you have been prescribed.
When making a request, we would be grateful if you would include any relevant information within the 'free text' box provided in the on-line form. For instance, you may want to tell us that you need a prescription earlier than usual because you are going on holiday.
Young adults:
We recommend young adults register independently for our on-line services. This will secure your confidentiality not only now but also in the future. If you have been registered by a parent in the past, please let us know and we will send you a new PIN number.
