Important: Completing the form doesn’t automatically make you a patient. You will receive a text message to confirm once your registration has been accepted.

Before You Register

We use a total triage system for appointments- please take a moment to read about how this works before registering.

To join our surgery you must:

  • Live in our boundary area. You can check this using the postcode tool below (sometimes one side of the street is included, but the other isn’t).
  • Complete the registration form carefully, including both your current and previous addresses. Incomplete forms will be rejected.
  • Registering a child? Please include full details for parent or guardians- incomplete forms can’t be processed.

  • New residents moving into Hemel Hempstead (within our catchment area)
  • Newborn babies whose parents are already registered with the practice
  • New residents of care or nursing homes where we already have registered patients
  • Patients already registered with another GP practice within our local area
  • Patients living outside our catchment area
