Healthier You Programme NHS Diabetes Prevention Programme
If you’re referred to the Healthier You NHS Diabetes Prevention Programme, you can join a 9-month program to help lower your risk of diabetes. You can choose to take part in person, with a small group from your community, or get 1-to-1 coaching through an app. The same program is provided no matter which option you choose.
Type 2 diabetes is a serious condition, but with the right support, about three out of five cases can be prevented or delayed. Staying at a healthy weight, eating well, and staying active all help. This program is here to boost your knowledge, skills, and confidence to make healthy lifestyle choices, including:
- Eating a healthier diet
- Getting more exercise
- Improving emotional wellbeing and managing stress
- Managing your weight
Tier 2 – Watford FC SHAPE UP
Shape Up is a free 12-week weight management course designed for men aged 18 to 65 years old with a BMI (Body Mass Index) of 30 or over, or for those from ethnic minority backgrounds with a BMI of 28 or over.
Participants meet up weekly for a 90 minute session, comprising of a 30 minute educational segment and a 60 minute physical activity session.
These two segments are delivered with the aim of improving participants’ healthy lifestyle knowledge, to ensure a sustainable weight loss rather than the typically short-term benefit of a diet-based programme.
Tier 2 and Tier 3 – My Healthy Weight and My Healthy Weight Plus
My Healthy Weight and My Healthy Weight Plus is a 14 week lifestyle programme designed to support behaviour change around food and activity. It is delivered by a team of experts who will support you throughout your journey. They offer sessions either online or face to face and always tailored to suit your individual needs and lifestyle.
